You should fill out an IDR form when making a submission of any kind (patentable invention, copyrightable software, biological material, etc.) to WiSys. For information on how to complete an IDR form, see the Disclosure Process.
Invention Disclosure Report Form (Rich Text Format)
Use this format if you wish to download the IDR. After downloading the IDR and opening it in MS Word or another word processing program, type your information directly into the form. Then send the IDR to your intellectual property manager through our secure email page.
